I hereby solemnly declare that:
1. I am desirous of performing voluntary work under the banner of Sarthak mentoring program in assisting Mentees in capacity-building through mentorship programmes.
2. I have full legal power and capacity to enter into and perform my mentorship obligations.
3. I shall at all times, stay committed to the goals and objectives of Mentoring Program.
4. I shall take part in the mentorship duties on a wholly voluntary basis.
5. I shall, at all times act in the best interests of Mentororing Program and the Mentee.
6. All the information provided by me to Mentoring program and the Mentee is true, correct and accurate.
7. I shall bear all phone call costs or data costs that are incurred by me through the course of the mentorship.
8. As a Mentor, I shall refrain from purchasing any books, resources or gifts for the Mentee and shall communicate any requirement for purchase for the Mentee within 24-48 hours to Sarthak.
9. I shall limit my interaction with the Mentee to the prescribed duration and frequency in accordance with the particulars of the mentorship programme, as may be specified by Sarthak.
10. I shall primarily use the curriculum, syllabus, teaching tools and educational resources as provided by Sarthak to carry out the mentorship programmes. If there are any additional tools that I wish to use for mentorship purposes, I shall disclose and share the same to Sarthak team.
11. I shall ensure that all my interaction with every Mentee is professional in nature and agree to respect the integrity and dignity of every Mentee. I shall not engage in any acts or omissions or conduct that may be construed as harassment or abuse (whether physical, mental, emotional or sexual or of any other nature) of the Mentee.
12. I shall not engage in any conduct that may be construed as initiating, facilitating, contributing or encouraging the development of a personal relationship with any Mentee.
13. I shall comply with any and all reporting and follow-up requirements of the Sarthak Mentoring Program.
14. I shall adhere to the overall goals and activity timeline recommendations of the Sarthak. I shall submit progress reports (if required) periodically.
15. I shall report any and all concerns I may hold regarding the well-being (whether mental, emotional or physical) of the Mentees within 48 hours from the arising of such concern to authorised persons from Sarthak.
16. I shall be bound by all codes of conduct, guidelines, instructions and other definitive documents (including the Terms of Service, Privacy Policy, Disclaimer and any other documents that may be developed and circulated by Sarthak) from time to time.
17. If my Mentee and I agree to meet in-person, we shall decide the venue of the mentorship via mutual consent and agreement. I shall inform about this meeting in advance to Sarthak.
18. I shall maintain any and all information pertaining to my Mentee(s) in the strictest confidence, not to be disclosed to any person or entity, apart from the Sarthak, for an indefinite period of time during and after the Mentorship. This Information includes all such information that may be sensitive or confidential in nature.
